MMS HC is a collaboration of organizations and individuals partnering to promote health and well-being in our communities. This Collaborative serves as the Community Leadership Team for MMS CHS’s Statewide Health Improvement Program (SHIP) grant, from the Minnesota Department of Health.
Our Mission includes partnering with local communities to promote, plan, encourage and support efforts to impact environmental change and enhance healthful living in Meeker, McLeod, and Sibley counties.
Our vision for success means having the local communities, schools, workplaces and health care working cross-collaboratively to improve access to physical activity and good nutrition in each of our communities.
